Patient Financial & Treatment Terms

This page is a plain-language summary of the Invoice of Dental Fees and Contract and the Agreement to Arbitrate that every patient signs before treatment at Pler Smile (New Face MD Dentistry, LLC d/b/a Pler Smile). It is provided so you know what to expect before you sign. It is a summary only — the signed contract is the binding document, and if anything here differs from it, the signed contract controls. Please read the full contract and ask your coordinator any questions before signing.

Deposits

  • Procedures involving veneers (porcelain or resin), implants, crowns, gingivectomy, or Invisalign require a one-time non-refundable deposit of $500.00, paid when you sign the contract. For the All-on-X program the deposit is 10% of the treatment total, which reserves your surgical week and is refundable within the first seven (7) days as described in the program terms.
  • The deposit is non-refundable in all circumstances — including if you cannot obtain medical clearance, even for reasons beyond your control. By signing, you also agree not to seek a chargeback of the deposit.
  • The deposit is valid for one (1) year. If your procedure does not happen within that year, an additional amount is required to proceed, and quoted prices may change. If the procedure is postponed for a medical condition, the deposit stays valid until you are medically cleared after the condition resolves.
  • No deposit is required for services such as teeth whitening, teeth cleaning, or a smile design evaluation.

Payment

  • The quoted amount covers only the procedures listed in your contract, plus what the contract states is included: anesthesia (general or local), pre-treatment and follow-up appointments, and the consultation.
  • Pre-operative labs and tests are included only if you have them done at Pler Smile; otherwise blood work, X-rays, medical clearance, prescriptions, and other ancillary services are at your own expense. Travel expenses are yours.
  • The full balance is due no later than one week before treatment. If it is not paid by then, the procedure is cancelled.
  • You must bring the credit cards used for payment to your appointment. If a photo of the card was not sent to your coordinator beforehand, the procedure may be cancelled without refund. Card processing fees are the patient's responsibility.
  • If someone else pays for your treatment, that payor must also sign the contract and becomes financially responsible under the same terms.
  • If you received a discounted price for two or more procedures on the same date, the discount is void if you fail to get medical clearance or to pay on time — you would then owe the full undiscounted price.

Third-party financing

  • If your procedure is financed, the financing agreement — including interest rates — is exclusively between you and the third-party lender, on the lender's terms.
  • If the credit line is in someone else's name, the practice requires a notarized letter authorizing the charge and describing the procedure, and may contact the cardholder to verify permission. If the cardholder does not pay, you remain responsible for completing payment.

Cancellations, rescheduling, and refunds

  • 7 days' notice: to cancel or reschedule your procedure (or, for veneer treatments, the first appointment), you must give formal notice at least seven (7) days in advance. You get one free reschedule, requested at least a week before the procedure.
  • If you cancel seven (7) or more days before the procedure, you are refunded the amount you have paid less the deposit.
  • Second veneer appointment (final cementation): if you cancel it at any time, you are entitled to a refund of only thirty percent (30%) of the amount due, less the deposit.
  • Refunds may be further reduced by costs already incurred with third parties (instruments, equipment, or the veneers themselves), and may take up to thirty (30) days to process.
  • Rescheduling fees: rescheduling with less than two (2) days' notice costs an additional $500.00; cancelling one day before or on the day of the procedure costs a $199.00 last-minute fee; cancelling a procedure that was already rescheduled costs $500.00 per cancelled rescheduled procedure.

Medical clearance and appointments

  • If your procedure requires it, medical clearance, lab results, and pre-procedure testing must reach the office at least 15 days before your procedure. Missing that deadline cancels your procedure date with no refund.
  • If you are found medically ineligible less than seven (7) days before the procedure — including for positive drug, alcohol, nicotine, or illicit-substance results — there is no refund; at the practice's discretion you may reschedule for when you are eligible.
  • Call the practice immediately before your procedure if you catch a cold, have flu-like symptoms, or have unhealed cuts, scrapes, or sores — these may require the dentist and anesthesiologist to reschedule.

Results and your responsibilities

  • The contract states plainly that results are not guaranteed. Payment is for the services provided, not for a specific outcome, and payment obligations apply regardless of the result. If you are not satisfied, contact the practice at (954) 354-2991 — and you agree to mediate any payment dispute before seeking a chargeback.
  • You must disclose your complete medical history. Any liability arising from withheld medical information is yours alone.
  • Treatment is elective and your free choice; the dentist will explain all medical risks before you decide.
  • Veneers, implants, and Invisalign require multiple appointments. If you abandon treatment after the first appointment, you do so by your own choice and against your dentist's advice.
  • In a medical emergency during a procedure, you agree to be transported to the nearest hospital and to undergo the care a physician recommends.

Your dentist

  • Your contract is with Pler Smile — which schedules, coordinates, and provides support staff — while your dentist is an independent contractor directly responsible for performing the procedure and managing your continuing care.
  • If your chosen dentist cancels, you may switch to another of the practice's dentists or reschedule; a dentist's cancellation does not entitle you to a refund.

Dispute resolution

  • The contract is governed by Florida law, with venue in Miami-Dade County, and it includes a separate Agreement to Arbitrate: most disputes are resolved by binding arbitration under the Florida Arbitration Code instead of a jury trial.
  • Two types of claims are excluded from arbitration: claims for medical negligence (as defined by Florida statute) and actions to collect a fee from you.
  • Arbitration uses a three-arbitrator panel (one selected by each side within 30 days, plus a neutral arbitrator). Each party pays its own arbitrator and attorney fees plus a share of the neutral arbitrator's costs; no attorneys' fees are awarded, and both sides waive consequential, special, and punitive damages.
  • The arbitration agreement includes a class action waiver — claims may be brought only individually — and applies retroactively to prior relationships with the practice.

What this contract does not cover

Clinical informed-consent forms, photo/media consent, and the HIPAA Notice of Privacy Practices are separate documents provided at the clinic — they are not part of this financial contract and are not summarized here.
